Continuing Education Application

Individuals and Organizations may apply for Continuing Education (CE) credits for educational events.

Please note that applications purchased less than 8 weeks away from the event date are subject to the standard fee, while those purchased 8+ weeks in advance are granted a discounted rate. All applications will be cross referenced for a completed purchase transaction.

Once purchased, please submit the following to: administrator@orthopticscanada.org.

• Completed application package forms
• A detailed activity program (including times, speakers, titles and objectives for each presentation) 
• A copy of your "Proof of Attendance" form for the activity

Once your application is received, the COC will evaluate the submission and provide you with a Statement of Accreditation with the number of core and non-core hours the activity is awarded. This Statement is to be included on the Proof of Attendance and in any advertising materials.

Please note: ONLY if you are unable to purchase a CE application via the website and pay by credit card, please email administrator@orthopticscanada.org to process payment via a PayPal invoice for the (non-refundable) CE application fee. Once received, a copy of the CE application will be emailed to you.
